DMCA Policy

LevForma LLC respects the intellectual property rights of others and expects users of our services to do the same. In accordance with the Digital Millennium Copyright Act of 1998 ("DMCA"), we will respond to valid notices of alleged copyright infringement and will take appropriate action.

Filing a DMCA Takedown Notice

If you believe that content on LevForma's website infringes your copyright, please send a written notice to our designated DMCA Agent containing all of the following:

• A physical or electronic signature of the copyright owner or an authorized representative
• Identification of the copyrighted work you claim has been infringed
• Identification of the material you claim is infringing, with sufficient detail for us to locate it (e.g., a specific URL)
• Your contact information: name, address, telephone number, and email address
• A statement that you have a good faith belief that the disputed use is not authorized by the copyright owner, its agent, or the law
• A statement, made under penalty of perjury, that the information in your notice is accurate and that you are the copyright owner or authorized to act on their behalf

Counter-Notification

If you believe that content you posted was removed or disabled as a result of a mistaken or misidentified DMCA notice, you may submit a counter-notification. Your counter-notification must include:

• Your physical or electronic signature
• Identification of the material that was removed and its location before removal
• A statement under penalty of perjury that you have a good faith belief the material was removed due to mistake or misidentification
• Your name, address, and telephone number, and a statement that you consent to the jurisdiction of the federal district court for your district

Upon receiving a valid counter-notification, we will forward it to the original complainant. If the complainant does not file a lawsuit within 10 business days, we may restore the removed content at our discretion.

Repeat Infringers

LevForma reserves the right to terminate, in appropriate circumstances, the accounts or access of users who are repeat infringers of others' intellectual property rights.
